Steps
- Take a photo of the blank form, one page at a time.
- Text it and ask what the form is for.
- Ask it to go box by box.
- Write your answers on the paper yourself. Do not text your Social Security number, card number or bank number.
- Ask what to bring or attach, and where the form goes when it is done.

What is this form asking for?
It is an enrollment form for the after school program. It asks for the child's name and birthday, your phone number, and one emergency contact. Want me to go box by box?
What is an emergency contact?
A person they can call if they cannot reach you. Write that person's name, phone number, and how they are related to the child.
Make it stick
- Ask before you write. A form is easier when you know what every box means.
